The whole team in one conversation about one pattern — the meeting that always runs long, the conflict nobody names, the handoff that keeps failing — with agreements the team writes and keeps.

Change sticks when it is practised. A weekly session over one to three months gives a leader or a team time to try something, see what happens, and adjust — at a lower rate per session.
